We accept most insurance plans and do our best to provide
accurate information as to our participation. However we
recommend that you call your insurance carrier to verify before
your visit if you have any concern. If your insurance plan
requires a referral, we ask that you request and track the
progress to ensure it has been completed. If you need help with
this process, please call our office and we will be happy to
time is held for you. We know your time is valuable and we make
every effort to run on time however unexpected circumstances may
occur. Please understand that if extra time is needed to provide
excellent care for a patient it can affect our schedule. We ask

Please call your
pharmacy at least 24 hours in advance when you need a
prescription refilled. This will allow sufficient time for the
pharmacy to contact our office for authorization, if necessary.
Routine refill requests should be made during office hours. An
annual visit is required for ongoing prescription refills.
